The Commander's Head is a key item in Quake II.
In Unit 8, the Hangar section, Bitterman has orders to close the main hangar bay doors. However, he finds that the door from the Outer Hangar to Inner Hangar is restricted by a nearby security scanner. A Tank Commander would have the necessary clearance. Bitterman must venture within the gruesome Research Lab to find a repair facility, where a heavily damaged and mostly non-operational Tank Commander lies on an operating table, with its detached head nearby. Bitterman steals the detached head, and the Tank Commander's body tries to react to what is happening, but is unable to prevent the theft. Bitterman can then return to the security scanner and use the Commander's Head to proceed to the Inner Hangar. Incidentally, Bitterman will encounter operational and combat-ready Tank Commanders later in the game, in Unit 9.
In the Playstation version of Quake II, the player visits Research Lab for the same reason, i.e. to obtain a Commander's Head so it can be used for security clearance. However, the Commander's Head is used in Defense Command to gain access to the Gravity Booster.
In the Ground Zero campaign, as Stepchild navigates the Base Complex of Unit 2, he must obtain the head of another Tank Commander to pass through a security scanner in Waterfront Storage. Unlike the dismantled and inactive Tank Commander found by Bitterman, this Tank Commander is fully operational at the time - however, it's possible for Stepchild to kill it by activating a laser security system.
