JOHNC99 is a Single Player and Multiplayer level. While the original release did not include a name, a slightly modified release was later made that included the name Tom's World. It is unknown if the latter version was made by Carmack, but there is no difference between the two versions beyond the name.

This level was released by Carmack as a way of explaining .MAP information, hoping for users to stick to the .MAP format as it does not change versions as often as .BSP files. As this was released prior to Beta3, this means that JOHNC99 is the fourth earliest Quake level that was released to the public.

The only levels that the public could play prior to this were the levels that were part of Qtest; DM1: Place of Two Deaths, DM2: Claustrophobopolis, and DM3: The Abandoned Base. The fifth earliest level, E1M1: the Slipgate Complex (as JRBASE1), was released with the QuakeEd source on May 18, 1996.

This map was designed to explain 3d geometry to designers, meaning it was kept highly simplistic. Each brush was its own cube, an additive brush. The level only had three entities; a normal player start, a Deathmatch start, and a light.

A March 24, 1996 reference exists to this level, stating that Carmack fixed an issue in this level where the player could get stuck on Buttons. These Buttons were removed from the version released to the public. It is likely this level was used as a test environment for Carmack.
